Job Title: Licensed Practical Nurse – Hospice
We are seeking a compassionate and skilled Licensed Practical Nurse – Hospice to join our interdisciplinary team dedicated to providing exceptional end-of-life care. The ideal Licensed Practical Nurse – Hospice will be a patient-focused healthcare professional who understands the importance of comfort, dignity, and emotional support during a patient’s final journey. As a Licensed Practical Nurse – Hospice, you will play a vital role in delivering quality care to patients and families facing terminal illness.
The Licensed Practical Nurse – Hospice will provide direct nursing care under the supervision of a Registered Nurse, in accordance with the hospice care plan. This includes administering medications, monitoring symptoms, documenting patient progress, and offering emotional support. The Licensed Practical Nurse – Hospice will collaborate with other members of the hospice team including physicians, social workers, chaplains, and aides to ensure comprehensive, compassionate care.
As a Licensed Practical Nurse – Hospice, you will be expected to assess patient needs, provide timely interventions, and educate families on the hospice process and caregiving techniques. The Licensed Practical Nurse – Hospice must be adept at communicating with patients and families during sensitive moments, delivering care that honors the patient’s wishes and enhances quality of life.
The Licensed Practical Nurse – Hospice will work in a variety of settings, including homes, inpatient hospice facilities, and nursing homes, adapting to each environment with professionalism and empathy. The Licensed Practical Nurse – Hospice must be flexible, reliable, and committed to upholding the values and mission of hospice care.
Requirements and Qualifications:
Current and valid LPN/LVN license
Graduation from an accredited practical nursing program
Minimum of 1 year of clinical nursing experience, preferably in hospice, palliative care, or home health
Strong understanding of pain management and symptom control
Excellent communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary team
CPR certification and valid driver’s license (if applicable)
Passion for hospice philosophy and end-of-life care
